1. Offer Details: This promotion provides 50% off three Hyperbaric Oxygen Therapy (HBOT) and Red Light Therapy (RLT) sessions. The original price of $525 is reduced to $263 for the duration of the campaign.
2. Booking Requirements: To schedule an appointment, patients must call our office at 602-864-0304. Appointments cannot be booked online for this promotional offer.
3. New Patient Form: All patients participating in this promotion are required to complete and sign the New Patient Form prior to their first appointment. The form will be provided by our office and must be submitted before the initial session.
4. Limited Availability: This offer is subject to availability. We recommend booking early to secure your appointment.
5. Non-Transferable & Non-Refundable: This promotion is non-transferable and non-refundable. The three sessions must be used by the same individual and cannot be shared or transferred to another person.
6. Expiration: All sessions must be completed within 90 days of purchase. Unused sessions after this period will be forfeited.
7. No Additional Discounts: This offer cannot be combined with any other promotions, discounts, or packages.
8. Eligibility: This promotion is open to new and existing patients. However, patients must meet any medical or safety requirements for HBOT and RLT, as determined by the provider. One deal per person.
